We are looking for a compassionate and dedicated LVN to provide high-quality patient care in the comfort of their own homes. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering exceptional patient outcomes.
**Responsibilities**
* Provide skilled nursing visits to patients in their homes, as prescribed by their physicians
* Conduct thorough assessments of patients' physical and emotional needs
* Develop and implement individualized care plans to meet patients' unique needs
* Collaborate with other healthcare professionals to ensure seamless care coordination
* Maintain accurate and complete medical records
**Qualifications**
* One year of clinical experience and one year of home health experience
* Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records
* Graduate of an accredited practical or vocational nursing program
* Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
* Must possess and maintain valid CPR certification while employed in a clinical role
* Must be a licensed driver who can travel to all business locations
