Home Health RN Part Time

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide skilled nursing visits to patients in their homes, including admission, case management, and follow-up visits.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Confer with physicians to develop and revise care plans based on patient assessments and progress.
- Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
- Monitor patient progress, report to physicians and Clinical Managers, and make adjustments to care plans as needed.
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
