Job Title: Pediatric Home Health Nurse

We are seeking a compassionate and skilled Pediatric Home Health Nurse to join our team at Angels of Care Pediatric Home Health. As a Pediatric Home Health Nurse, you will provide high-quality, patient-centered care to children and young adults in their homes, making a meaningful difference in their lives.

Job Summary:

As a Pediatric Home Health Nurse, you will be responsible for providing direct, one-on-one medical care to infants, children, and young adults in their homes. Your duties will include conducting ongoing patient assessments, administering prescribed medication and treatments, coordinating care, educating family members on patient clinical care, and promoting preventative initiatives to protect quality of care.

Responsibilities:
  • Provide direct, one-on-one medical care to infants, children, and young adults in their homes
  • Conduct ongoing patient assessments and develop individualized care plans
  • Administer prescribed medication and treatments as ordered by physicians
  • Coordinate care with other healthcare professionals and family members
  • Educate family members on patient clinical care to enhance positive outcomes
  • Promote preventative initiatives to protect quality of care
Requirements:
  • Active RN or LPN/LVN license
  • Ability to work in a client home setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
