Job Title: Per Diem Pharmacist

Penn State Health is seeking a highly skilled and dedicated Per Diem Pharmacist to join our team. As a Per Diem Pharmacist, you will play a vital role in ensuring the safe and effective use of medications for our patients.

Job Summary:

The Per Diem Pharmacist will be responsible for providing pharmaceutical care to patients, including medication therapy management, patient education, and medication reconciliation. You will work closely with our interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.

Responsibilities:
  • Provide pharmaceutical care to patients, including medication therapy management and patient education
  • Conduct medication reconciliation and ensure accurate medication lists
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
  • Participate in quality improvement initiatives to enhance patient care and safety
  • Stay up-to-date with the latest pharmaceutical knowledge and guidelines
Requirements:
  • PharmD or BS in Pharmacy degree
  • Current Pennsylvania licensure or eligibility
  • Previous hospital pharmacy experience preferred
  • Excellent communication and teamwork skills
