Patient Care Assistant

Position Overview:
The Patient Care Assistant is responsible for delivering essential daily nursing services to assigned patients, adhering to their individual care plans and under the guidance of the nursing supervisor.
Core Responsibilities:
1. Ensure the safety and well-being of patients/residents.
2. Manage the application and removal of restraints, and facilitate physical exercises.
3. Assist patients with mobility, including transfers from bed to wheelchair, using proper techniques and equipment.
4. Support patients with personal hygiene tasks, including bathing and grooming.
5. Aid in dressing and undressing, as well as providing oral care and skin maintenance.
6. Accompany residents to activities and events as needed.
7. Assist in meal service, ensuring dietary needs are met and documenting food intake accurately.
8. Participate in patient admissions, discharges, and transfers, while understanding care plans.
9. Maintain cleanliness of medical equipment and assist with catheter care and incontinence needs.
10. Offer hydration and manage the distribution of fresh water to patients.
11. Document patient information and changes in condition according to established protocols.
12. Report any concerns regarding patient care or conditions to the supervising nurse.
13. Conduct range of motion exercises for patients.
14. Provide care for bedridden patients, ensuring they are turned regularly.
15. Identify and report any signs of abuse or changes in patient condition to the appropriate personnel.
16. Change and maintain bed linens as per schedule or need.
17. Measure and record vital signs, documenting any relevant health information.
Required Skills and Qualifications:
- Excellent communication skills with patients and their families, fostering a welcoming environment.
- Ability to respond compassionately to patient concerns.
- Professional demeanor when addressing inquiries.
- Participation in mandatory training and educational programs to stay updated in the field.
- Adherence to patient rights and compliance with safety regulations.
- Capability to perform assigned duties while following established safety protocols.
- Flexibility to undertake additional responsibilities as directed by the nursing supervisor.
