As a Patient Services Coordinator, you will play a critical role in ensuring the timely and efficient delivery of patient care services.
Key Responsibilities- Coordinate Patient Assessments: Ensure that patient assessment visits, including Oasis visits, are scheduled and performed in a timely manner.
- Optimize Clinician Scheduling: Work collaboratively with field clinicians to ensure that they are assigned and scheduled in the most efficient geography, maximizing clinician efficiency and optimizing clinician capacity.
- Manage Scheduling Issues: Manage calls from patients and field staff related to scheduling issues and refer clinical and performance-related issues to the clinical manager.
- Communicate with Patients and Clinicians: Support clinical manager and work collaboratively to ensure timely communication with patients, clinicians, referral sources, business development team, and other office personnel.
- Run Reports and Respond to Work Flow: Run all applicable reports and respond to work flow, taking appropriate actions to ensure the smooth delivery of patient care services.
- Assist with Patient Transfers: Assist with internal or external transfer of patients between care centers and/or hospice services.
- High School Diploma or GED Equivalent: A high school diploma or GED equivalent is required for this role.
- Experience in Scheduling or Healthcare Administration: One year of scheduling experience in a healthcare environment or one year of administrative experience in a healthcare environment is preferred.
