We are seeking an experienced Patient Linkage Coordinator to connect patients with essential medical care services and support. The ideal candidate will possess strong communication skills, be able to work effectively with high-risk populations, and have experience in case management and counseling.

Key Responsibilities:
  • Coordinate patient referrals, financial screenings, and first medical visits within our organization.
  • Network with community agencies and organizations to establish referral linkages and provide comprehensive support services.
  • Provide benefit enrollment, financial screening, case management, and counseling support until patients successfully link to medical care.
  • Collaborate with nursing and psychosocial case management teams to ensure continuity of care after the first medical visit.
  • Document all interactions and outcomes using our electronic medical records system.
Requirements:
  • A valid California Class C Driver's License.
  • At least 1 year of experience in HIV or health/human service fields.
  • Bachelor's degree in a related field or equivalent work experience.
  • Basic HIV-Test Counselor Certification.
  • Working knowledge of word processing (MS Word) and Excel programs.
