At National Health Foundation, we are seeking an exceptional Medical Coordinator LVN to join our Recuperative Care Program. As a key member of our team, you will play a vital role in providing post-hospital care and creating a healthy, safe, and home-like environment for our guests.

The ideal candidate will have excellent bedside manners, be a team player, and possess a 'whatever it takes' attitude. You will work closely with our guests, colleagues, and healthcare partners to ensure seamless transitions and outstanding patient care.

Responsibilities
  • Facilitate hospital discharge process and patient assessment/engagement to program.
  • Perform medical assessment intake and discharge plan of care.
  • Maintain current guest chart via existing CRM database.
  • Conduct medical/wellness checks, vital signs monitoring, medication regimen education, preventative measures, and medical insurance follow-up.
  • Coordinate and provide linkages for guest's medical appointments and follow-up.
  • Collaborate with Intake to ensure appropriate eligibility to recuperative care services and admission process, including procurement of required DMEs and Home Health needs.
  • Participate in weekly internal and external case conferencing, board review team meetings, monthly staff meetings, trainings, and in-services.
  • Communicate with insurance, hospital partners, and/or other referring parties on plan of care, guest progress, and guest's needs as appropriate.
  • Provide advocacy on guest behalf when needed and in tandem with LVN manager, Nurse Practitioner Administrator, or Director's instructions.
  • Recognize, manage, and respond to emergency medical conditions.
  • Track and complete guest information on existing database, as well as any required platform, specific to progress and plan of care.
  • Perform administrative tasks such as charting, guest summaries, and answering phones.
