Patient Care Coordinator RN, Virginia Hospital Center

What You'll Do:

This critical role is responsible for coordinating patient care from admission through discharge, ensuring efficient and effective transitions between care settings. As a Patient Care Coordinator RN, you will work closely with the multidisciplinary team to develop and implement personalized care plans, promote timely discharges, and enhance the overall patient experience.

Key Responsibilities:

  1. Develop and maintain relationships with patients, families, and healthcare providers to facilitate informed decision-making.
  2. Assess patients' discharge needs and create tailored care plans to ensure safe, timely transitions.
  3. Coordinate with various stakeholders, including social workers, case managers, and community resources, to address complex patient needs.
  4. Monitor care progress, identifying opportunities for improvement and collaborating with the team to implement changes.
  5. Communicate proactively with patients and their families to educate them about their care plans, goals, and expected outcomes.
