What You'll Do:
This critical role is responsible for coordinating patient care from admission through discharge, ensuring efficient and effective transitions between care settings. As a Patient Care Coordinator RN, you will work closely with the multidisciplinary team to develop and implement personalized care plans, promote timely discharges, and enhance the overall patient experience.
Key Responsibilities:
- Develop and maintain relationships with patients, families, and healthcare providers to facilitate informed decision-making.
- Assess patients' discharge needs and create tailored care plans to ensure safe, timely transitions.
- Coordinate with various stakeholders, including social workers, case managers, and community resources, to address complex patient needs.
- Monitor care progress, identifying opportunities for improvement and collaborating with the team to implement changes.
- Communicate proactively with patients and their families to educate them about their care plans, goals, and expected outcomes.
