RN Transition Manager

**Job Description**
This is an exciting opportunity for a skilled and compassionate RN to join our team at Virginia Hospital Center in Arlington, Virginia. As a Utilization Review RN, you will be responsible for assessing patient care needs, coordinating transitions of care, and ensuring seamless hospital-to-home or community-based services.
Responsibilities
- Conduct thorough assessments of patient care needs and develop effective care plans
- Collaborate with hospitalists and other healthcare professionals to ensure timely and appropriate discharge planning
- Coordinate with insurance companies and other stakeholders to verify eligibility and benefits
- Identify and mitigate potential barriers to care, including delays in throughput and variance from quality standards
- Promote high-quality, patient-centered care through effective documentation and communication
Requirements
- Bachelor's degree in Nursing (BSN) preferred
- Minimum one year of experience as an RN in an acute care setting
- Active licensure as a Registered Nurse in Virginia within six months of hire
- Ability to pass background check and drug screen
- Excellent communication and collaboration skills, with the ability to work effectively in a fast-paced environment
