Position Overview:
We are seeking a

Patient Care Manager

to become part of our dedicated team.
You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager.

Your responsibilities will include overseeing the daily operations of your designated hospice location, managing the clinical components of the hospice program, and ensuring high-quality care for our patients.


Sign-On Bonus: $5,000.00
In the absence of the Executive Director, Administrator, or Senior/Executive Patient Care Manager, you will manage the site.
Oversee clinical, administrative, and recordkeeping functions.
Respond to phone referrals and inquiries with professionalism and compassion.
Participate in the on-call nursing schedule and provide direct skilled nursing care in patients' homes.

Candidate Profile:

Education and Experience:
Registered Nurse with at least an Associate's Degree in Nursing from an accredited institution is required.

Unless specified otherwise in state-specific requirements
A Bachelor's Degree in Nursing is preferred from an accredited institution.
A minimum of three years of direct patient care experience in a clinical environment is required. Unless specified otherwise in state-specific requirements
At least one year of management or supervisory experience is required.
Two or more years of experience in hospice or home care is required.
Current nursing license in the state of employment is mandatory.

Licenses, Certifications, and Registrations:
Current nursing license in the state of employment
CPR Certification
Valid driver's license and current automobile insurance

Personal Attributes and Skills:
Empathy towards the feelings of terminally ill patients and their families before and after the patient's passing.


Ability to demonstrate adaptability by performing various tasks, often transitioning between different duties without losing efficiency or composure.

Communicate effectively with individuals from diverse socioeconomic backgrounds. Influence others' opinions, attitudes, or judgments regarding ideas or matters. Make evaluations or decisions based on sensory or judgmental criteria. Exhibit evidence of patient advocacy. Display responsible behavior in the workplace, maturity, cooperation, flexibility, and tact when interacting with colleagues and all members of the hospice team.

Specialized Knowledge and Skills:


Understanding of the needs of terminally ill patients and their families, along with a comprehensive grasp of the hospice concept; knowledge of the roles of all disciplines involved in providing hospice services; exceptional patient assessment abilities; strong oral and written communication skills; thorough understanding of managed care principles and regulatory guidelines (i.e., Medicare, Medicaid, and human resource management principles).
