Key Responsibilities
The primary duty of the Patient Transition Specialist is to ensure a smooth transition for patients moving from a facility setting to the care of an LHC Group hospice agency.
- Accountable for meeting admission goals as defined at the time of hire or during the annual review of agency budget objectives.
- Assists the LHC Group agency in preparing for and accepting the care of the patient following discharge.
- Supports the Administrator in executing contracts for facility-based services for hospice patients.
- Communicates hospice services and agency protocols to patients and their family members.
- Engages family members and caregivers in the educational process and evaluates post-discharge educational needs.
- Participates in bi-weekly interdisciplinary group meetings, as needed, to provide updates on accounts, customer requirements, and progress towards agency growth strategies.
Experience Requirements
- Minimum of one year of home health experience or one year of hospital case management experience is required.
- Preferred one to three years of experience in medical marketing.
- Current RN or LPN or SW or PT licensure in the state of practice is required.
- Certification from a technical school demonstrating strong clinical knowledge is necessary.
- Must possess reliable transportation and maintain a current driver's license and auto insurance.
- Exceptional verbal and written communication skills with all members of the healthcare team are essential.
- Strong organizational skills and the ability to manage competing priorities are required.
- Comprehensive understanding of home health qualifying criteria and coverage guidelines is necessary.
- Proficient computer skills are required.
- Excellent presentation, negotiation, and relationship-building skills are essential.
- Strong computer skills to meet Microsoft Outlook and other software requirements are necessary.
- Ability to work independently with minimal supervision and be self-motivated is required.
