This role is a full-time, on-site position for a Registered Nurse Case Manager at Gentiva Hospice. The responsibilities include, but are not limited to, discharge planning, management of healthcare resources, nursing care, and overall case management to facilitate the complete care of our patients.

Key Responsibilities

  • Implement discharge planning and utilization management strategies
  • Provide nursing and medical care with a focus on patient needs
  • Engage in effective communication and interpersonal interactions
  • Collaborate with a multidisciplinary team to enhance patient outcomes
  • Experience in hospice or palliative care is advantageous

Qualifications

  • Current RN license in the state of Tennessee
  • Preferred: BSN or related educational background
  • Demonstrated skills in case management and utilization management
  • Strong foundation in nursing and medical practices
  • Ability to work collaboratively in a team-oriented environment
