Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Georgia Community Support and Solutions, Inc. (GCSS) dba InCommunity. As a Licensed Practical Nurse, you will play a vital role in providing nursing activities, supports, care, and treatment to children and adolescents with autism spectrum disorders and intellectual disabilities.

Job Summary:

The primary function of this position is to perform nursing activities, supports, care, and treatment to children and adolescents in accordance with the policies of GCSS and the State of Georgia Nursing Licensing Bureau. You will work closely with an interdisciplinary team, including BCBA, Psychiatrist, Psychologist, and other related service providers, to ensure the best possible outcomes for our clients.

Responsibilities:
  • Serves as the lead staff of the home, responsible for the management of the unit, and ensuring the unit is clean and equipment is in good working condition.
  • Coordinates nursing care for individuals, contacts doctors as needed, and completes entire shifts as assigned.
  • Provides skilled nursing care, completes health assessments and evaluations, administers and supervises the administration of medication, and ensures the daily activity schedule is followed.
  • Responsible for the daily management of the unit during assigned shifts, promptly reports to Registered Nurse and family of changes in the individual's condition, and ensures the unit is clean and equipment is in good working condition.
  • Coordinates nursing care for individuals, contacts RN and doctor as needed, reviews doctor's orders, supervises and directs the activities of direct care staff and RBTs, and ensures the client is in a safe and productive environment at all times.
  • Problem-solves issues and concerns as they arise in the home, communicates with pharmacy to order medications for individuals as needed, and ensures monitoring of medications for individuals receiving services.
  • Participates in staff meetings, meets with the Registered Nurse on a routine basis, assists with the interviewing, hiring, and training of employees, and assists with performance appraisals and disciplinary actions.
  • Maintains highly organized, consistent, thorough, and systematic recordkeeping practices, reviews and audits direct care staff records to ensure compliance, and provides consultation regarding crisis intervention and critical incident supports.
  • Completes incident reports within 24 hours of incident, monitors house funds, collaborates with Registered Nurse, Program Manager, Case Manager, and Director of Clinical & Crisis Support Services to ensure the homes are managed well and run efficiently, and participates in professional growth activities.
Qualifications:

To perform this job successfully, an individual must be able to perform each essential job function satisfactorily. A minimum of two years of experience serving individuals with developmental disabilities and a minimum of three years of recent experience working with individuals who have autism spectrum disorders and their families are required. Graduation from an accredited School of Nursing is also required.
