About the Role

We are seeking a dedicated and compassionate LPN to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will provide one-on-one care to medically fragile patients in their homes, promoting their health and well-being.

Key Responsibilities
  • Implement and manage patient care plans in accordance with physician orders
  • Conduct thorough patient assessments and coordinate care with healthcare teams
  • Administer medications, treatments, and therapies as prescribed
  • Document patient care and progress in a timely and accurate manner
  • Provide education and training to patients and their families on health promotion and disease management
  • Collaborate with healthcare teams to ensure seamless care transitions
Requirements
  • Active LPN license in good standing
  • Current CPR certification
  • TB skin test (current within last 12 months)
  • 6 months prior experience in home health care or a related field (preferred)
  • Reliable transportation
