Job Summary

Avante at St. Cloud, Inc. is seeking a skilled Social Services Assistant to provide critical support to the Social Services department. This role is responsible for assisting with discharge planning, PASRR Level II paperwork, and various social services office functions.

Key Responsibilities
  • Assist with discharge planning, including arrangements for home health services, DME, transportation, and post-discharge follow-up calls.
  • Complete PASRR Level II paperwork in a timely and accurate manner.
  • Perform various social services office functions, such as filing, faxing, phone calls, and mailing.
  • Assist with room changes and document in the medical record.
  • Coordinate with Activities staff to obtain personal items, clothing items, and adaptive equipment as needed.
  • Assist with the ICP Medicaid process, including applications for Medicaid.
  • Complete assigned social services audits to ensure compliance with state and federal regulations.
  • Assist with complaints or formal grievances filed by residents and/or responsible parties.
  • Coordinate and assist with departmental referrals, including dental, eye care, hearing, podiatry, psychiatry, and psychological support.
Qualifications
  • Must be able to read, write, and communicate in English.
  • Must be computer proficient with applicable programs, such as Microsoft Word, Excel, Outlook, and Giftwrap.
Education and Experience
  • High School Diploma required.
  • Must have at least 3 years of experience in a related field.
  • Long Term Care experience preferred.
