Senior Primary Care Physician Assistant

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Abode Care Partners, a leading provider of integrated medical services. Our team delivers high-quality care to individuals from post-hospitalization to home in various settings.
About the Role:
- Deliver evidence-based care to patients with complex conditions in Assisted Living and Home Health settings.
- Perform thorough medical evaluations and collaborate with healthcare professionals to develop comprehensive patient care plans.
- Maintain accurate patient records and scheduling.
Responsibilities Include:
- Providing primary care to seniors and patients with chronic illnesses.
- Conducting medical evaluations and developing treatment plans.
- Collaborating with caregivers and other healthcare professionals to ensure interdisciplinary care.
- Ordering and interpreting laboratory and diagnostic testing as needed.
Requirements:
- Active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
