We are seeking a highly skilled Primary Care Patient Advocate to join our team at CenterWell Senior Primary Care. In this role, you will be responsible for delivering exceptional patient care and advocating on behalf of our patients to ensure they receive the best possible outcomes.

The Role

The Primary Care Patient Advocate will work closely with our interdisciplinary care teams to provide comprehensive care to our patients. This will involve conducting transitions of care management, providing triage guidance and supportive consultation to other team members, developing care plans leveraging the 5Ms Geriatric best practice framework, and identifying existing barriers to engagement with necessary resources and supports. You will also serve as a liaison between the patient and direct care providers, assisting in navigating both internal and external systems.

Key Responsibilities

* Conduct Transitions of Care Management for a subset of the patient population
* Provide triage guidance and supportive consultation to other team members
* Develop care plans leveraging the 5Ms Geriatric best practice framework
* Identify existing barriers to engagement with necessary resources and supports
* Serve as a liaison between the patient and direct care providers

Requirements

To be successful in this role, you will need to have a Registered Nurse (RN) license and a minimum of 4 years of experience working in human services and navigating community-based resources. Familiarity with state Medicaid guidelines and application processes is preferred, as is experience working with patients with behavioral health conditions and substance use disorders. You should possess advanced clinical acumen, ability to multi-task in a fast-paced work environment, flexibility to fluidly transition and adjust in an evolving role, excellent organizational skills, advanced oral and written communication skills, strong interpersonal and relationship building skills, compassion, and a desire to advocate for patient needs.

Compensation and Benefits

This role offers a competitive salary of $69,800 - $96,200 per year, based on experience and qualifications. Location and Schedule

This role is based in Indianapolis, Indiana, and requires regular travel to patients' homes, treatment facilities, and community-based settings. The schedule is Monday through Friday, 8:00 AM to 5:00 PM, with overtime available as needed. You must reside in the Indianapolis metro area.
