Position Overview

The Intensive Care Coordinator plays a crucial role in managing and responding to mental health crises among children and adolescents. This position requires a compassionate individual who can effectively navigate challenging situations while ensuring the well-being of consumers.

Key Responsibilities

Clinical Duties

  • Timely response to crisis calls to ensure immediate support.
  • Coordinate the Child/Adolescent Crisis Team's response to urgent situations.
  • Recognize crisis states and apply de-escalation techniques effectively.
  • Assess the required level of response in crisis scenarios and prioritize actions accordingly.
  • Maintain clear communication with Access to Care for seamless service delivery.
  • Provide crisis intervention as necessary.
  • Utilize the CANS (Child and Adolescent Needs and Strengths) tool to assess consumer needs.
  • Identify objectives and formulate plans for suitable services.
  • Facilitate connections to community resources for comprehensive support.
  • Monitor consumers to ensure placement in the least restrictive settings.
  • Conduct ongoing evaluations of consumers' progress and needs as identified in the CANS.
  • Advocate for consumers to secure necessary community resources.
  • Collaborate effectively with treatment team members and external partners in case management.
  • Evaluate consumers for discharge planning purposes.
  • Track clinical outcomes in accordance with contractual obligations.
  • Demonstrate proficiency in child/adolescent-specific competencies.

Supervision and Professional Development

  • Seek clinical supervision and consultation as required.
  • Accept constructive feedback and implement suggestions for improvement.
  • Actively pursue opportunities to enhance case management skills.

Documentation and Record Keeping

  • Ensure timely documentation in line with AltaPointe policies.
  • Complete comprehensive CANS assessments accurately.
  • Document specific and measurable goals in individualized service plans.
  • Maintain clear and concise records of consumer issues, ISP goals, and services rendered.
  • Record case dispositions, closures, follow-ups, and referrals appropriately.
  • Ensure legibility in all documentation.

Caseload Management

  • Manage caseload effectively based on the needs of consumers.
  • Meet productivity standards set by AltaPointe.

Qualifications

A Bachelor's degree in a mental health-related field is required. Candidates must be at least 23 years old, possess a valid driver’s license, and maintain a clean driving record.

Knowledge in child/adolescent mental health specialties is essential and should be acquired within six months of employment, including:

  • Understanding psychiatric disorders prevalent in the child/adolescent demographic.
  • Familiarity with the growth and development stages of children and adolescents.
  • Awareness of developmental disabilities and mental health challenges in younger populations.
  • Basic understanding of behavior management and parenting strategies.
  • Knowledge of cultural diversity and its implications in mental health.
  • Proficiency in crisis intervention techniques and non-violent crisis management.
  • Awareness of the impacts of abuse and neglect on mental health.
  • Knowledge of community resources available for child/adolescent support.
  • Certification from the Alabama State Department of Mental Health for case management services.
  • Familiarity with the CANS assessment tool.
  • Experience in facilitating behavioral health therapy groups for various age groups.
