The Role
Position: Supported-Living Assistant Program Coordinator
Location: Wakefield, MA
Pay rate: $24.50 per hour
Hours: 40 hours per week, days and hours flexible depending on needs of Individuals and Program

The Assistant Program Coordinator responsibilities include management of the individual records, including the coordination of required weekly reports from Life Skills Mentors and the documentation of the individuals ISP and the Mentor Support Plan. The Assistant Program Coordinator will provide and/or coordinate daily living support to adults with developmental disabilities to help them live as independently as possible, using the home and community as the classroom. Manage the 118 Nahant St and the individuals residing there. This is a leadership role.
- Ensure that all required paperwork and training is appropriately completed and submitted in a timely manner. This includes but is not limited to ISP documentation, weekly progress notes, and other documentation as needed for all In-Home Supports Individuals.
- Assist mentors to prioritize areas of emphasis for everyone. Areas include, but are not limited to, medical, social, spiritual, cultural, recreational, nutritional, financial, and educational support.
- Assist in the completion of assessments and other paperwork as assigned by the Program Coordinator or other management staff. Will access HCSIS to complete Incident Reports, ISP documents, and Healthcare Records for 118 Nahant St and as needed for the other individuals
- Will be On-call for the In-Home Supports programs in absence of the Program Coordinator
- Participate in trainings and meetings as required.
- Will serve as the Human Rights and Fire Safety Officer for In-Home Supports and conduct trainings for staff and individuals on an annual basis
- Will conduct host Our Health Matters, Craft Night, and similar groups to promote socialization.
- Ensure program files are maintained and updated as needed. for 118 Nahant St.
- Will work directly in conjunction with the families and service coordinators for the individuals at 118 Nahant St
- Monitor and facilitate progress of ISP goals for all individuals including 118 Nahant St.
- Responsible for attending all medical and counseling appointments for the individuals at 118 Nahant.

- Assist individuals at 118 Nahant St. in developing maximum independence (budgeting, time management, use of leisure time, laundry, transportation, etc.)
- Oversee the health, nutrition, social and general welfare of program participants for 118 Nahan St
- Completion of weekly mentor reports for designated individuals at 118 Nahant St.
- Bachelor's Degree in Education or Human Services preferred or 3 years' experience supporting individuals with developmental disabilities.
- Experience working with families of individuals and professional service providers.
- Current Medication Administration Certification required.
- Current CPR and First Aid Certifications required.
- Must write and speak fluent English.
- A passion and dedication to supporting our people is a must
- Valid Driver's license, reliable transportation, and acceptable driving record
- Must pass company background and reference checks
The employee must occasionally lift 30 pounds alone, or up to 50 pounds with assistance of another person or adaptive equipment while performing the duties of the job. The employee is regularly required to talk or hear. The employee frequently is required to stand, use hand to feel, reach with hands, arms, climb or balance and stoop. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
Work Environment:
While performing the duties of this job the employee must drive to various appointments and spend time in various homes of individuals supported. The noise level in the work environment is usually moderate.
