La Causa, Inc. is seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in coordinating services for children with severe emotional problems and their families.
Key Responsibilities- Facilitate Services: Facilitate all aspects of services for participants, including transportation, assembling child and family teams, assessing needs and strengths, developing individualized Plans of Care (POCs), advocating, and providing or securing services as needed.
- Liaison and Communication: Serve as a liaison between the child/family and community service providers, ensuring the acquisition of services and maintaining good communication, collaboration, and cooperation with all stakeholders.
- Documentation and Reporting: Prepare, complete, and submit required documents, including Progress Notes, POCs, Crisis Plans, and Service Authorization Requests (SARs), by due dates.
- Professional Development: Attend studies (QA/QI), meetings, and professional development activities, as directed.
- Team Player: Demonstrate a team player attitude and perform other duties as assigned.
- Education: Bachelor's Degree from an accredited school in Social Work or a related field (i.e., psychology, sociology, counseling).
- Experience: One (1) year of continued experience providing mental health services.
- Bilingual: Bilingual, Spanish and English, required for bilingual positions.
- Skills: Strong cultural competency and interpersonal relationship skills, strong communication skills, written and verbal, and excellent critical thinking and organizational skills.
- Transportation: Valid Wisconsin Driver's license, state minimum auto insurance, and meet La Causa, Inc. driving standards.
- Background Checks: Must successfully complete and pass all background checks.
Work is generally performed in field and office environments, including travel. Local travel required; some state-wide travel as needed. Flexible hours required; as dictated by program needs.
