Job Overview

We are seeking an experienced Home Health Registered Nurse to join our clinical team. The ideal candidate will be a skilled and compassionate nurse, with a strong background in home care and a commitment to delivering high-quality patient care.

Key Responsibilities
  • Admit new clients to the agency and complete recertifications/discharges as necessary, utilizing OASIS assessments.
  • Complete routine client visits, providing personalized care and support.
  • Participate in the on-call team, sharing supervisory responsibilities with other RNs and participating in the contingency plan for 24/7 clients.
  • Perform other duties as discussed or assigned.
Requirements
  • Graduate of an accredited nursing program with a current license as a Registered Nurse in the state of Colorado.
  • Minimum of two (2) years nursing experience in home care, and at least one (1) year of supervisory experience.
  • Demonstrated ability to supervise, coordinate, and direct clinical personnel.
  • Knowledge of Medicare/Medicaid and ACHC standards preferred.
  • BLS certified.
  • Excellent written and verbal communication skills and strong interpersonal skills, including the ability to market and relate tactfully with clients and other members of the community.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or Agency requirements.
