About the Role:

In this role, you will be responsible for providing direct patient care, including assistance with daily living activities, personal care, and mobility support. You will also coordinate homecare services, ensuring that patients receive the necessary care and support. To be successful in this role, you will need to have excellent communication skills, be able to work independently, and have a strong understanding of patient care principles and practices.


Key Requirements:
  • A high school diploma or equivalency, successful completion of a Board of Nursing-approved training program, and current listing with the division of Facility Services Registry.
  • One year of patient care experience in an acute care or custodial setting.
  • Documented proof of at least 75 certified education hours.
  • Ability to independently organize work and provide effective healthcare services to homebound patients.
  • CPR certification and working knowledge of safe and appropriate personal care practices.
