Job Description:

Job Summary:

As a Home Health/Hospice Registered Nurse with HexaQuEST Health, Inc., you will play a vital role in delivering high-quality patient care in the comfort of their own homes. You will assess patient needs, develop and implement individualized care plans, and provide ongoing supervision to ensure the best possible outcomes.

Key Responsibilities:

  • Conduct comprehensive assessments of patient needs and develop personalized care plans in accordance with evidence-based practice, state and federal regulations, and VNAHSR policies.
  • Implement medical plans of care, nursing interventions, and procedures to ensure safe and effective patient care.
  • Collaborate with healthcare teams, including physicians, providers, and vendors, to coordinate patient care and ensure seamless transitions.
  • Provide patient and caregiver education to empower informed decision-making and promote self-care.
  • Document patient care and outcomes accurately and thoroughly, adhering to agency policies and procedures.
  • Participate in peer review audits and contribute to quality improvement initiatives.
  • Stay up-to-date on industry developments, best practices, and regulatory requirements to maintain expertise and provide exceptional patient care.

Requirements:

  • 2+ years of experience in home health or hospice care.
  • Current RN licensure in the state of Vermont.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Strong analytical and problem-solving skills.

Working Conditions:

This is a full-time position with a normal schedule, Monday-Friday, 8am-4pm, with occasional on-call or weekend shifts required.
