Job Summary:

HexaQuEST Health, Inc. is seeking a skilled Home Care/Hospice Registered Nurse to join our team. As a Home Care/Hospice RN, you will provide high-quality patient care in the comfort of their own homes. This is a rewarding opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:

• Assess patients' needs for HCA services and develop and coordinate a plan of care with the patient and their caregiver.
• Provide on-going supervision and support to patients and their caregivers to ensure the highest level of care and comfort.
• Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
• Conduct thorough assessments and develop individualized plans of care that address the unique needs of each patient.
• Provide education and support to patients and their caregivers on disease management, medication management, and other health-related topics.
• Maintain accurate and up-to-date records of patient care and progress.

Requirements:

• Current RN licensure in the state of Vermont.
• 2+ years of experience in home care or hospice nursing.
• Strong assessment and communication skills.
• Ability to work independently and as part of a team.
• Strong attention to detail and organizational skills.
