Job Description:

As a Home Health/Hospice Registered Nurse with HexaQuEST Health, Inc., you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Your expertise will be essential in assessing patient needs, developing and implementing personalized care plans, and providing ongoing supervision to ensure the best possible outcomes.

Key Responsibilities:
  • Conduct comprehensive assessments of patient needs to identify areas for improvement and develop effective care plans.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their caregivers to promote informed decision-making and self-management.
  • Stay up-to-date with the latest evidence-based practices, technologies, and regulations to ensure the highest standard of care.
  • Document patient progress and outcomes accurately and efficiently to inform future care decisions.
  • Participate in peer review audits and quality improvement initiatives to drive continuous improvement.
Requirements:
  • 2+ years of experience in home health and/or hospice care.
  • Current RN licensure in the state of Vermont.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills.
Work Environment:

This is a full-time position with a normal shift schedule, Monday-Friday, 8am-4pm. Some on-call or weekend shifts may be required. The ideal candidate will be able to work effectively in a fast-paced environment and adapt to changing priorities.
