Job Responsibilities
- Collects patient and family data and reports findings to supervisor in a timely manner.
- Assists in the communication of information to assigned patients.
- Under the supervision of the Staffing Manager, performs interventions and procedures as directed to ensure the safe effective quality of care for patients.
- Reports to supervisor on a variety of aspects of patient and family information, including identified problems, care provided, and patient's responses to meet patient and family needs and ensure overall quality of care delivered.
- Maintains up-to-date and accurate documentation of care provided to ensure the effective integration of information for use by the health care team to ensure on-going and continued quality of care.
- Advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs.
- Carries out a variety of household chores, including sweeping, mopping, laundry, doing dishes, cleaning bathrooms, vacuuming, simple meal preparation, and serving.
- Ensures that tasks are performed to patient satisfaction.
- Stays up to date on Homemaker 'Do's and Don'ts' requirements.
- Proactively reports changes in skin conditions to RN, Case Manager, or program manager in a timely manner.
Qualifications
- High School Diploma or GED
- Valid Driver's License and Current Auto Insurance
- Must be at least 18 years of age
- Weekend and holiday rotation required

- Eligible employees that have been with the VNAHSR for one year and worked a minimum of 1000 hours can have their LNA course and materials completely paid for, and will receive a raise upon receipt of certification.
