Job Summary:


Clackamas County's Public Health Office of Public Health Emergency Services is seeking a Community Paramedic to support the delivery of medical services and connect patients to primary care and other resources.


Responsibilities:



  • Collaborate with community partners on projects such as responses to opioid use and overdoses, behavioral health matters, addressing frequent 911 users, and community engagement to increase access to care.
  • Perform the scope of practice of a paramedic, administer patient care consistent with protocols, medical direction, and Public Health policies.
  • Examine, screen, treat, and coordinate health services for patients, including initial in-home assessments and ongoing home visits.
  • Conduct post-hospital release follow-up care, including general physical exams, history and physical exams related to diagnosed conditions, daily or weekly weight checks, medication accuracy review, dressing changes, and checking vital signs.
  • Observe, record, and report to physicians patient conditions, reactions to drugs, treatment, and significant incidents.
  • Conduct patient education on diabetes prevention/treatment, hypertension, congestive heart failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), pneumonia, fluid intake, nutrition, substance use disorder, and fall prevention.
  • Perform post-injury follow-up, including fall prevention activities such as securing rugs and carpets, assessments for grab bar placements.
  • Coordinate appointments and follow-ups with physicians and hospitals.
  • Connect individuals to community resources.
  • Complete required reports and documentation, compile and use records, reports, and statistical information for appraisal and planning of assigned duties.
  • In emergency situations, support Public Health Department Operation Center (DOC), Clackamas County Emergency Operation Center (EOC), and other department emergency operations and responses.

Requirements:



  • Minimum of three years of related experience as a practicing Paramedic providing the required knowledge and skills to perform the responsibilities of this position.
  • Oregon Paramedic License (required at time of hire).
  • Preferred special qualifications include Community Paramedic (CP-C) Certification, Critical Care Paramedic (CCP-C) Certification, Oregon Qualified Mental Health Associate (QMHA) Certification, Oregon Community Healthcare Worker Certification, Case Management experience working with allied health, county health, and social services, and Bilingual English/Spanish proficiency.
  • Pre-employment requirements include passing a post-offer, pre-employment drug test, criminal history check, post-offer physical assessment, and having a valid driver's license with an acceptable driving record.
