About the Role

We are seeking a highly skilled and compassionate Healthcare Manager - Geriatric Care Specialist to join our team at East Boston Neighborhood Health Center (EBNHC). As a key member of our Home Care Department, you will play a vital role in ensuring the delivery of high-quality PACE home care services to our patients.

Responsibilities
  • Provide direct supervision to a team of health aides, ensuring their performance meets the highest standards.
  • Assist in policy review and revision, ensuring consistency with practice, changes in regulatory requirements, or other updates.
  • Maintain accurate records and reports through QAPI or other avenues as required.
  • Conduct periodic audits to ensure Personal Service Plans are up-to-date and aligned with current clinical and functional status.
  • Participate in the annual competency review and training program, ensuring that training tools are updated and relevant.
Requirements
  • Bachelor's degree in Nursing or equivalent experience/certification.
  • Current MA Nursing License.
  • Strong clinical background in geriatric care, with expertise dealing with elders and families.
  • Home healthcare management experience preferred.
