We are seeking a highly skilled and experienced MDS Nurse Coordinator to join our team at Trinity Health. As a key member of our nursing staff, you will play a vital role in ensuring the delivery of exceptional care to our residents.
Key Responsibilities- Lead and coordinate the development and completion of comprehensive resident assessments in accordance with state regulations, OBRA guidelines, and the goals of our facility.
- Ensure accurate case mix and payment determinations (PPDM) through meticulous attention to detail and adherence to established procedures.
- Collaborate with the Director of Nursing and other healthcare professionals to develop and implement effective care plans.
- Provide education and training to nursing staff on MDS best practices and OBRA guidelines.
- Stay up-to-date with changes in regulations and guidelines, and implement updates as necessary.
- Must possess a Nursing Degree from an accredited college or university or be a graduate of an approved RN program.
- Must hold a current, unencumbered, active license to practice as a Registered Nurse in this state.
- Extensive knowledge of nursing and medical practices, as well as relevant laws, regulations, and guidelines for long-term care.
- Familiarity with OBRA and case mix systems is crucial.
- Must possess and maintain current CPR Certification. AANAC Certification is required or must be obtained within 6 months of hire.
- Prior experience with MDS is mandatory, with a strong emphasis on OBRA assessments and PPDM/case mix management.
