Job Title: Population Health Specialist

La Pine Community Health Center is seeking a highly skilled Population Health Specialist to join our team. As a key member of our quality improvement team, you will play a critical role in identifying gaps in patient care and developing strategies to improve clinical outcomes.

Key Responsibilities:
  • Audit clinical records and reports to identify patients who need preventive services, diagnostics, and follow-up.
  • Generate and enhance quality improvement reports to support QI initiatives.
  • Proactively review provider schedules and notify care teams of opportunities for patients with upcoming appointments.
  • Outreach to patients directly to discuss preventative care needs and arrange follow-up with providers.
  • Exercise motivational and coaching techniques as necessary to remove barriers to care and engage patients in their care.
  • Update the medical record to reflect completion of recommended health maintenance and procedures.
  • Collaborate with billing department and provide technical assistance to staff on use of data and reporting systems to drive performance improvement.
  • Participate in the evaluation of team data for proactive panel management and serve as a quality improvement team member for grants and special projects.
Requirements:
  • Possess excellent communication skills and ability to learn quickly in a rapidly growing and changing work environment.
  • Strong attention to detail and desire to problem solve, with a keen interest in data collection, analysis, and reports.
  • Exceptional customer service skills and ability to work independently with good judgment in a variety of situations.
  • Intermediate level computer skills and ability to use Microsoft Windows: Word and Excel.
  • Strict adherence to HIPAA laws and regulations concerning confidentiality and security of protected health information.
Preferred Qualifications:
  • Bachelor's Degree or medical certification.
  • Experience with Epic EHR and knowledge of quality measures including: QIM, HEDIS, and UDS.
  • Experience in panel management and ability to work in an office environment.
Working Conditions:

This position may be exposed to airborne and blood-borne pathogens, and hazardous materials. This position is not eligible to be a telecommute position.
