Clinical Nurse Navigator

The RN Navigator for the Hepatobiliary Program is a registered nurse who utilizes their clinical expertise to guide patients and their families through the healthcare continuum. This role assumes responsibility for planning, guiding, and evaluating the delivery of patient care, ensuring compliance and optimum care throughout treatment.
Key Responsibilities:- Collaborate with a multidisciplinary team to facilitate timely and appropriate screening, diagnosis, therapeutic intervention, treatment, and supportive care.
- Coordinate all resources necessary to provide care, utilizing advanced expertise to identify and implement process improvements.
- Provide patient education regarding disease process and treatment throughout the healthcare continuum.
- Maintain a professional, safe, and therapeutic environment for rendering care.
- NYS licensed/registered (or eligible) RN required.
- Bachelor's degree in Nursing or healthcare-related field, three (3) years related RN experience required (preferably in specialty) OR an Associates degree in Nursing and five (5) years related RN experience (preferably in specialty) required.
- Excellent written/oral communication and interpersonal skills required.
- CPR-BLS certification required by the time of appointment.
- Three (3) years as a Certified Nurse/Patient Navigator in a specialty area preferred.
- Acute care RN experience preferred.
