The Department of Veterans Affairs is seeking a highly skilled Registered Nurse to join our team as a Women's Health Cervical Care Navigator. This is a full-time position that requires a strong commitment to providing exceptional patient care and promoting health and wellness in our veteran population.
Key Responsibilities- Provide direct gender-specific Veteran care and care coordination
- Manage assigned panel(s) and ensure appropriate evaluation and access is provided to patient panels
- Provide gender-specific health promotion, health education, and coaching on wellness, disease prevention, and chronic care management
- Formulate a plan of care based on nursing assessments and seek guidance on how to improve care provided to patients and families
- Delegate elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE)
- Ability to formulate a plan of care based on nursing assessments and seek guidance on how to improve care provided to patients and families
- Ability to delegate elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient
- Experience as an RN in a similar role or setting
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Knowledge of health promotion, health education, and coaching principles
