Care Coordinator Specialist

">About the Role:">

This position is responsible for supporting tenants in maintaining housing stability. The ideal candidate will have a strong understanding of community resources and be able to provide linkages and referrals as needed.

The Independent Living Specialist, now known as the Care Coordinator Specialist, makes regular visits to tenant apartments to ensure a safe and secure environment. This role requires a deep understanding of the needs of individuals experiencing homelessness and the ability to provide supportive counseling.

  • Assist tenants with applications and re-certification for entitlements">
  • Provide linkages and referrals to community resources">
  • Make regular visits to tenant apartments">
">Requirements:">

A minimum of 3 years relevant experience and HS diploma/GED required. Experience working with the substance abuse and/or mentally ill population required.

">Estimated Salary:">

$55,000 - $65,000 per year, depending on experience.
