About the Role:

  • Conduct home visits to assess patient needs and develop comprehensive care plans
  • Collaborate with interdisciplinary teams to coordinate patient care
  • Provide skilled nursing services, including wound care, injections, and medication management
  • Evaluate patient progress and adjust care plans as needed
  • Maintain accurate and timely documentation of patient interactions and care provided

Requirements:

  • Graduate of an approved school of professional nursing
  • Current licensure as a Registered Nurse in New York State
  • Minimum 1 year of experience in home health or hospice care
  • Excellent communication and interpersonal skills
