About the Role

We are seeking a skilled Clinical Rehabilitation Navigator to join our team at Ascension St. Vincent's Southside Hospital. As a key member of our inpatient rehabilitation team, you will play a vital role in ensuring the seamless transition of patients from admission to discharge.

Key Responsibilities
  • Establish and maintain relationships with internal and external referral sources to drive patient admission volume in the inpatient rehab unit.
  • Conduct clinical chart assessments and on/off-site evaluations to ensure appropriate patient selection and admission to the rehab unit.
  • Develop and implement business plans for the rehab unit, contributing to the growth and development of referral source promotional information.
  • Provide educational opportunities and resources to patients, families, caregivers, and healthcare professionals.
Requirements
  • Licensure/Certification: One or more of the following required: Registered Nurse (RN) credentialed from the Florida Board of Nursing, or Rehab Therapist specializing in Physical Therapy (RPT), Occupational Therapy (OTR), or other relevant certifications.
  • Education: High School diploma equivalency with 2 years of cumulative experience, or Associate's degree/Technical degree, or 4 years of applicable cumulative job-specific experience.
