Job Summary

We are seeking a skilled Clinical Rehabilitation Navigator to join our team at Ascension St. Vincent's. As a key member of our inpatient rehabilitation team, you will play a critical role in ensuring seamless communication between internal and external referral sources, patients, families, and healthcare professionals.

Key Responsibilities

  • Establish and maintain relationships with referral sources, case managers, physicians, and payors to drive patient admission volume in inpatient rehabilitation.
  • Conduct clinical chart assessments and on/off-site evaluations to ensure appropriate patient selection for inpatient rehabilitation services.
  • Develop and implement business plans for the rehabilitation unit, including referral source promotional materials.
  • Provide educational resources and support to patients, families, and caregivers.

Requirements

  • Licensure/Certification/Registration: One or more of the following required:
  • Registered Nurse credentialed from the Florida Board of Nursing.
  • Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Florida Board of Physical Therapy Practice.
  • Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Florida Board of Occupational Therapy.
  • BLS Provider preferred.
  • Education: High School diploma equivalency with 2 years of cumulative experience or Associate's degree/Technical degree or 4 years of applicable cumulative job-specific experience required.
