Job Summary

At Woodland Heights Medical Center, we are seeking a skilled Care Coordinator RN to join our team. As a key member of our interdisciplinary care team, you will be responsible for coordinating the overall plan of care for patients from admission to discharge.

Key Responsibilities

  • Act as a liaison between patients, families, and healthcare personnel to ensure timely and effective care.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Monitor patient progress and make adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare staff to ensure seamless care transitions.

Requirements

  • Associate degree required; bachelor's degree preferred.
  • Current RN licensure and at least 2 years of clinical experience.
  • Familiarity with standard concepts, practices, and procedures within a healthcare setting.
