Service Coordinator

We are seeking a highly skilled Service Coordinator to join our team at the Texas Department of Aging & Disability Services.
As a Service Coordinator, you will be responsible for determining the needs and eligibility of individuals for Community Care services. This involves conducting home visits, interviewing individuals, documenting information, and developing service plans.
Your primary objective will be to ensure that individuals receive the necessary support and services to meet their needs. You will work closely with community agencies, healthcare providers, and other stakeholders to coordinate services and ensure seamless delivery.
This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Key Responsibilities:
- Conduct home visits and interviews to assess the needs of individuals for Community Care services.
- Develop and implement individualized service plans to meet the needs of individuals.
- Maintain accurate and up-to-date records of individual service plans and progress.
- Collaborate with community agencies and healthcare providers to coordinate services and ensure seamless delivery.
- Monitor and evaluate the effectiveness of service plans and make adjustments as needed.
Requirements:
- Bachelor's degree in a relevant field, such as social work or a related field.
- 2+ years of experience in a related field, such as social work or case management.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of community resources and services.
Preferred Qualifications:
- Master's degree in a relevant field, such as social work or a related field.
- Experience working with vulnerable populations, such as older adults or individuals with disabilities.
- Knowledge of relevant laws and regulations, such as the Americans with Disabilities Act.
