The Head of Social Services will oversee the Social Services Department's operations, ensuring compliance with federal, state, and local regulations, as well as company policies. This role is crucial in helping residents and their families navigate transitions, health challenges, and care planning to achieve optimal well-being.
Key Responsibilities:
- Uphold the personal and property rights of all residents while performing duties, including respecting privacy.
- Adhere to all company and departmental policies and procedures.
- Conduct assessments for residents upon admission, quarterly, and when there are changes in condition to identify social service needs. Ensure comprehensive psychosocial histories and assessments are completed for each resident.
- Collaborate with the interdisciplinary team to develop and maintain individualized care plans.
- Support residents and families during the transition to facility living.
- Determine residents' legal status upon admission, gather advance directives, and ensure that treatment preferences are honored in accordance with company policies.
- Provide individual, family, and group services aimed at enhancing psychosocial functioning and addressing emotional and behavioral challenges.
- 2-3 years of relevant experience; supervisory experience is preferred.
- Experience in a skilled nursing environment.
- For facilities with 120 beds or more, a Bachelor's degree in social work or a related field is required; for smaller facilities, a BA degree is preferred.
- Commitment to delivering high-quality, person-centered care with dignity, respect, and compassion.
- Ability to represent the organization professionally and ethically.
- Effective communication skills to interact with residents, families, and team members.
- Capacity to work respectfully with individuals facing various physical and mental health challenges.
