At ISHR, we are seeking a skilled Social Services Professional to join our team. As a key member of our care team, you will be responsible for conducting social work assessments to determine the psychosocial needs, preferences, and goals of our participants.
Key Responsibilities
Conduct social work assessments to identify participant needs and develop care plans
Collaborate with the primary care physician and other care team members to ensure smooth care transitions
Deliver and document social work interventions, including counseling and case management
Facilitate care conference meetings to enhance care coordination among care team members, participants, and their support network
Requirements
Masters Degree in Social Work (MSW) required
Minimum of one (1) year of experience with a frail or elderly population
Previous experience coordinating and facilitating care conference meetings
Preferred previous experience assisting individuals with behavioral health and substance abuse issues
