About the Role

We are seeking a dedicated and compassionate Direct Support Professional to join our team at Brooklyn Community Services. This is a full-time position, working 35 hours per week, Monday to Friday from 8:30am to 4:30pm.

Job Summary

The successful candidate will be responsible for supervising a group of Day Habilitation participants as they engage in activities on and off site. They will ensure that the group adheres to their weekly activity sheet and exhibit appropriate social behavior during participation in the program.

Key Responsibilities
  • Supervise a group of Day Habilitation participants
  • Ensure adherence to weekly activity sheets
  • Incorporate life and job skills into group activities
  • Foster personal empowerment through person-centered planning
  • Attend to participants' personal care
  • Complete daily checklists and monthly summary notes
