Job Overview

Seastar Senior Services is a pioneering senior care company in Portland, Oregon, dedicated to providing exceptional in-home care, companionship, and care management services to seniors and vulnerable adults in the Portland metropolitan area and surrounding towns.

We are seeking a professional Care Manager to join our team, who is passionate about delivering high-quality, compassionate care to our clients. As a Care Manager, you will be responsible for performing comprehensive assessments, gathering information about health history, family and social dynamics, natural support systems, and environmental concerns and needs.

Key Responsibilities:

  • Conduct thorough assessments to identify client needs and develop personalized care plans.
  • Implement care plan actions to achieve established goals and objectives.
  • Update assessments and care plans quarterly or as needed.
  • Respond promptly to changes in client condition or environment.
  • Coordinate medical appointments, specialty follow-up visits, procedures, and obtain necessary medications.
  • Arrange for transportation to appointments and social outings.
  • Document all work performed on client's behalf with detailed notes and accurate time.
  • Participate in weekly on-call rotation for evenings and weekends.

Requirements:

  • Registered Nurse License REQUIRED.
  • Experience working with seniors and adults with disabilities.
  • Excellent driving record.
  • CPR certified or able to obtain.
  • Good observational and writing skills.
  • Excellent listening skills with critical thinking.
  • Computer skills required.

Compensation:

$50.00 per hour DOE.

Job Types: Part-time, Per diem.

Expected hours: 5 - 10 per week to start, will increase as clients onboard.

We do drug tests and nationwide background checks.
