Care Management and Coordination Services
The Registered Nurse Clinical Case Manager (RNCCM) provides care management and care coordination services for at-risk patients, including patients with complex medical and psychosocial issues. This position is a core member of the patient-centered medical home (PCMH) teams led by the physician/provider.
Key Responsibilities:
- Apply population-based, chronic care management approaches and clinical protocols to provide high-quality care.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Coordinate care services to ensure seamless transitions and optimal patient outcomes.
- Provide education and support to patients and their families to promote self-management and empowerment.
Requirements:
- Graduation from an accredited school of nursing.
- Oregon Registered Nurse License.
- 3 years of clinical experience as an RN.
Preferred Qualifications:
- Bachelor's Degree in Nursing.
- 2 years of home health, skilled nursing facility, and/or community health nursing.
- 2 years of managed care experience and/or utilization management experience.
- 2 years of care/case management experience.
