Job Summary

As a RN-Care Manager at Providence Medical Group, you will play a vital role in coordinating care for our most medically fragile patients. You will work closely with patients, families, and caregivers to develop and implement personalized care plans that promote self-management and improve health outcomes.

Key Responsibilities
  • Coordinate care for patients with complex medical needs, including those with multiple chronic conditions and functional impairments.
  • Develop and implement care plans that address the physical, emotional, and social needs of patients and their families.
  • Collaborate with healthcare providers, community resources, and social services to ensure seamless transitions of care.
  • Provide education and support to patients and families to promote self-management and independence.
  • Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
Requirements
  • Oregon Registered Nurse License upon hire.
  • 2 years Clinical medical-surgical, acute care or long-term care experience that also includes: 1 year in community-based/ambulatory care. 1 year in care of the frail elderly.
Preferred Qualifications
  • Bachelor's Degree in Nursing.
  • Home health, hospice, palliative care, managed care, or skilled nursing facility background.
  • 1 year Care management of diverse patient populations.
