Position Overview: Clinical Support LPN
*This is an office-based position
Full-Time Schedule
Day Shifts (7:30 AM - 4:30 PM or 8:00 AM - 5:00 PM)
No Weekend or Holiday Work
Job Summary
The Clinical Support LPN collaborates with Clinical Support RNs to manage incoming calls from our 24-hour helpline and acts as a vital resource for our home hospice clinical staff.
Key Responsibilities
- Respond promptly to all clinical inquiries.
- Assess and triage calls, focusing on pain management, physical needs, and disease progression.
- Provide guidance to callers or coordinate with RNs for necessary follow-up, prioritizing urgent cases.
- Ensure interdisciplinary team visits for new symptoms, deaths, or crises.
- Follow up on after-hours calls and client concerns as needed.
- Apply palliative care protocols for symptom management in collaboration with RNs.
- Facilitate the initiation of medication management orders through timely communication with pharmacies.
- Obtain medication refills, renewals, and new orders in partnership with RNs.
- Document services accurately on the day they are provided, including prognosis, care levels, and changes in care plans.
- Maintain electronic patient records meticulously.
- Communicate effectively with staff, physicians, patients, families, and community agencies.
- Inform other disciplines and volunteers about changes in patient conditions and care plans.
- Educate patients and caregivers on diagnoses, treatments, and end-of-life expectations.
- Understand the implications of reimbursement and payer sources on care plans.
- Assist in orienting new staff members.
Qualifications
- Current Pennsylvania nursing license is required.
- Preferred: One year of recent medical/surgical nursing experience as a licensed practical nurse.
