Position: Clinical Support LPN

Type: Office-Based Role

Schedule: Full Time

Hours: Days (7:30am - 4:30pm or 8am - 5pm)

Note: No weekends or holidays

JOB SUMMARY

The Clinical Support LPN collaborates with Clinical Support RNs to manage incoming calls from our 24-hour service line, acting as a vital resource for home hospice clinical staff operating in the field.

ESSENTIAL FUNCTIONS

- Responds promptly to all clinical inquiries.
- Assesses calls to evaluate pain levels, symptoms, physical requirements, and disease progression.
- Provides guidance to callers or coordinates with RNs for necessary follow-up, prioritizing urgent cases involving uncontrolled pain or imminent death.
- Ensures interdisciplinary team visits for new symptoms, deaths, or crises.
- Follows up on after-hours calls, visits, and client concerns as needed.
- Demonstrates knowledge and application of palliative care protocols for effective symptom management in collaboration with RNs.
- Initiates system management orders by promptly notifying the contracted pharmacy.
- Obtains medication refills, renewals, and revised orders in partnership with RNs.
- Communicates with provider offices regarding outstanding or unclear orders.
- Accurately documents services provided on the same day, supporting prognosis, care levels, plan changes, disease progression, and care needs.
- Maintains electronic patient records diligently.
- Effectively communicates with organizational staff, physicians, patients, families, nursing home personnel, external contractors, and community agencies.
- Updates other disciplines and volunteers about changes in patient conditions, goals, and care plans.
- Educates patients and caregivers on diagnoses, care procedures, treatments, medications, and end-of-life expectations.
- Understands the implications of reimbursement and payer sources on care plans.
- Assists in orienting new staff members.

QUALIFICATIONS

- Current Pennsylvania nursing license is required.
- Preferred: One year of recent medical/surgical nursing experience as a licensed practical nurse.
