Position: Clinical Support LPN

Type: Office-Based Role

Schedule: Full Time

Hours: Days (7:30 AM - 4:30 PM or 8:00 AM - 5:00 PM)

Weekends and Holidays: Not Required

Job Overview

The Clinical Support LPN collaborates with Clinical Support RNs to manage incoming calls from our 24-hour helpline and serves as a key resource for our home hospice clinical staff.

Key Responsibilities

  • Respond promptly to all clinical inquiries.
  • Assess pain, symptoms, physical needs, and disease progression during call triage.
  • Provide guidance to callers or work with RNs for necessary follow-up, prioritizing urgent cases.
  • Coordinate interdisciplinary team visits for new symptoms, death, or crises.
  • Follow up on after-hours calls and client concerns as needed.
  • Implement palliative care protocols for symptom management in collaboration with RNs.
  • Facilitate timely communication with contracted pharmacies for medication management.
  • Ensure accurate documentation of services provided, prognosis, care levels, and changes in care plans.
  • Maintain electronic patient records meticulously.
  • Communicate effectively with various stakeholders including staff, physicians, patients, and families.
  • Educate patients and caregivers on diagnoses, treatments, and end-of-life expectations.
  • Understand the implications of reimbursement and payer sources on care plans.
  • Assist in orienting new staff members.

Qualifications

Applicants must possess a current nursing license in Pennsylvania. A minimum of one year of recent medical/surgical nursing experience as a licensed practical nurse is preferred.
