Physician Assistant or Nurse Practitioner

At Bluestone Physician Services, we're seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our team of dedicated healthcare professionals. As a key member of our care team, you'll provide on-site primary care to patients residing in Assisted Living and Memory Care communities in and around Shoreview, MN.
About the RoleThis is a full-time position that offers a unique opportunity to design and manage your schedule, ensuring a healthy work-life balance. You'll work closely with our multidisciplinary care teams to deliver preventative, proactive, and tailored care to patients. Our model of care has been shown to result in 21% fewer ER visits, 36% fewer hospitalizations, and 41% fewer hospital readmissions compared to patients with similar conditions and complexities.
Responsibilities- Provide on-site primary care to patients in Assisted Living and Memory Care communities
- Manage a panel of 200 patients, with an average of 12-16 patients per day
- Design and manage your schedule to ensure a healthy work-life balance
- Develop and implement care plans that meet the unique needs of patients
- Collaborate with community partners, including home care, hospice, and other resources as needed
- Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
- Board Certified/Board Prepared through AANP, ANCC, or NCCPA
- Licensed or ability to obtain licensure through the Minnesota Board of Nursing or Board of Medical Practice
- Ability to obtain and/or maintain DEA certification for the State of Minnesota
- Proven ability to communicate effectively with strong verbal skills
- Excellent interpersonal and customer service skills
