As a Hospice Registered Nurse Case Manager, you'll play a vital role in leading with care, compassion, and skill to deliver top-notch hospice services. Your expertise will make a significant difference in the lives of others, upholding our high standards of nursing practice.

Key Responsibilities:
  • Maintains a specified case management load, ensuring seamless patient care and family support.
  • Conducts initial and ongoing assessments to determine hospice needs, focusing on pain and symptom management.
  • Participates in on-call duties, providing around-the-clock support for our patients and families.
  • Develops and revises care plans in collaboration with the Interdisciplinary Group (IDG), patients, families, and attending physicians.
  • Coordinates care among IDG members, healthcare providers, and other stakeholders to ensure continuous patient needs assessment and implementation of the IDG plan of care.
  • Evaluates and coordinates services according to the IDG plan of care, promoting patient-centered care.
  • Supervises and directs LPN/LVN, Hospice Aide, and Homemaker services, ensuring high-quality care delivery.
  • Facilitates consideration of alternative care levels, such as inpatient, respite, or continuous care, as indicated by patient status and needs.
Requirements:
  • Graduate of an accredited school of nursing.
  • Licensed RN in South Carolina, with active license in good standing.
  • Twelve months of accumulated experience as a registered nurse within the last five years or current clinical knowledge as deemed appropriate by the DHCS.
  • One year of hospice care experience, with precepting or training experience preferred.
  • CPR certification and a valid driver's license with auto liability insurance.
